It%u2019s important to know that move-ins generally do not happen over night, but rather take time to gather information, sometimes weeks. With some private-pay communities, a deposit will hold your space. At others, there is no financial transaction but moving forward with paperwork is the next step. Even with a move from a hospital for rehab or long-term care takes time to process necessary details. The documentation that you will need to move into an independent living community or even a skilled nursing facility will vary but there are still common documents for you to find, request, or prepare. Below is a starting point of possible information needed. From the prospective resident or their family:%u2022 Basic demographics form.%u2022 Photo identification (if available). Driver%u2019s license front and back photo copy will often be requested. %u2022 Social Security card.%u2022 Insurance card(s).%u2022 Documentation from area employer programs or military service. %u2022 Financial information. %u2022 DPOA, Patient Advocate, or Guardianship paperwork. DNR paperwork if applicable. %u2022 COVID vaccination card showing all vaccinations and boosters. Note: Vaccination may not be required, but facility is often required to document residents%u2019 vaccination status.%u2022 If the prospective resident is currently in another skilled nursing facility, a copy of current Minimum Data Set (MDS) should be requested from the facility.From a physician:%u2022 History and physical, including height and weight, dated within the last 60 days.%u2022 List of current medications, with dosages.%u2022 Diet order if exists. %u2022 Mental Health Screening.%u2022 Copy of any recent lab work.%u2022 TB results.Once information has all been received, an in-person assessment meeting may be scheduled prior-to admission. The prospective resident%u2019s medical condition and respective care needs will be reviewed to ensure the facility can meet needs. A nursing home must also conduct a nursing facility level of care determination to assess the resident%u2019s functional ability for Medicaid to pay for nursing home care. Financial commitments and supporting documents as well as various insurance information will be reviewed prior to official acceptance as a resident. A discussion about additional items of cost and care will be supplied and discussed. Once that is complete and the admission is accepted, you will be updated on a timeline for admission. In some cases, you may be placed on a waiting list and the facility will reach out when a room that meets your needs and requests is available. Once you are notified of an available room or bed, most long-term care facilities will not hold a bed longer than five days. Don%u2019t fret however, if you are not ready within that time period to move in, you can choose to remain on the waiting list until another appropriate room or bed is available.
