Part of their time may be spent in the day facility and then the balance of their day at home. To qualify for PACE, you must meet conditions: %u2022 Medically qualified, Medicaid LTC eligibility.%u2022 55 or older, and live in the service area.%u2022 Able to live safely in the community with PACE support. The PACE program becomes the sole source of services for Medicare and Medicaid eligible enrollees. Individuals can leave the program at any time. To find out if you%u2019re eligible and if there%u2019s a PACE site near you, visit pace4you.org, npaonline.org, or Medicaid.gov. A list of local programs can be found on page 17 under the Adult Day Care / PACE listing. Adult Care ResidenceAdult Foster Care (AFC) homes are licensed and inspected by the state. AFC homes provide room and board, special diets, supervision, and some personal care to adults who may be frail but in generally good health. Personal care may include help with bathing, dressing, and taking medications. Some AFC homes specialize in care for older adults, individuals with a developmental disability, or individuals with a mental illness. AFC homes range from 20 residents in a large group home to six or less in a family home. Most AFC homes are private pay and do not accept Supplemental Security Income (SSI) as full payment. Residents pay for their care with Social Security, pensions, other income, and savings. %u201cAssisted Living%u201d is designed for aging adults who need gentle assistance with activities of daily living such as dressing, bathing, or medication reminders. Assisted living offers a private apartment, dignified care, and peace of mind that medical assistance is available round the clock by pulling a cord or pushing a call button.Each apartment is generally equipped with a kitchenette as well as a private bath with walk-in shower and safety bars. Some communities offer various sized apartments such as studio, and one or two bedrooms. Social and recreational activities are offered daily. Three meals are provided each day tailored to special dietary requirements. Special group outings are generally offered along with community rooms, hair salons, exercise rooms, libraries, gift shops, and other special amenities such as transportation to medical appointments. Some even offer in-house physical and occupational therapy. %u201cAssisted Living%u201d is a term used to describe a kind of residential care program.
