  INSPIRE SLEEP APNEA DEVICE
By: Steven Szeles, MD, Philip Mataverde, DO, Zachary Richens, DO, Doug Diemer, PA
Obstructive sleep apnea (OSA) is a serious con- dition that affects millions of people worldwide. It is characterized by disrupted breathing due to a collapse of the upper airway during sleep, which can lead to snoring, awakening at night, excessive daytime sleepiness, difficulty concentrating, and memory problems. Continuous Positive Airway Pressure (CPAP) is the most common treatment for OSA, but some people cannot tolerate it. For those who are not responding well to CPAP thera- py, Inspire therapy is a minimally invasive surgi- cal treatment option available to certain patients.
Bronson Otolaryngology, Head & Neck Surgery Specialists work together with representatives from Inspire to evaluate, diagnose, and treat patients with OSA. A shared web-based applica- tion called SleepSync tracks each patient’s prog- ress through the entire process.

Inspire is a hypoglossal nerve stimulator that is surgically implanted under the skin of the chest and neck. It consists of a small device the size
of a pacemaker, a stimulation lead and a sensor to monitor breathing patterns during sleep. The device senses inhalation and then delivers voltage to the nerve, which helps keep the upper airway open and allows for normal breathing.
Fine tuning sleep study: Once your sleep spe- cialist decides the best settings for you, a fine-tun- ing sleep study is done to see how well the device helps to stop the airway from collapsing. Your sleep specialists will use this sleep study to deter- mine additional fine turning of the settings.
Inspire was FDA approved in 2014 and is the only FDA-approved obstructive sleep apnea treat- ment that works from within the body and is con- trolled by a remote.

sleep specialists and otolaryngologists are highly experienced at providing Inspire Bronson’s sleep specialists and otolaryngologists are highly expe- rienced at providing Inspire therapy as a treatment for OSA. Otolaryngologist Dr. Steven Szeles
has been at the forefront of this groundbreaking technology, having participated in Inspire clinical trials from 2011 to 2012. His involvement in its development helped mark a major milestone in the field of sleep health. Dr. Szeles currently performs the procedure at Bronson Battle Creek Hospital and Bronson Methodist Hospital.
Who can have Inspire sleep apnea treatment? Inspire is an alternative treatment for those who cannot or will not use CPAP or other proven methods such as a mouthpiece. In addition, candidates for Inspire therapy need to be:
• At or below a body mass index of 32-35kg/m3
Drug Induced Sleep Endoscopy (DISE): Before the procedure, your ENT surgeon will do a DISE, which uses a small camera in the nostrils to visualize the collapsible portions of your upper airway. This is done in an operating room under anesthesia. The vast majority of patients will have a front to back collapse pattern which is respon- sive to Inspire therapy. If the airway collapses on all sides then Inspire will not be approved.

sleep apnea.
• Free of concentric collapse of the upper airway
The surgery: Those who qualify for Inspire will undergo a minor outpatient surgery lasting approximately two hours to implant the device under the skin in the chest. A second small inci- sion is made in the neck where the stimulation cuff is attached to the hypoglossal nerve. The device is tested before the surgery is complete but
determined during a drug induced sleep endos- copy performed by the implanting ENT surgeon prior to surgery.

Initial consultation with a sleep health specialist: Your sleep provider will review your medical history; discuss your sleep apnea symp- toms and any past testing and treatments you have received. At this consultation, you may be asked questions about your personal medical history and sleep habits, which will help your provider decide if Inspire treatment is right for you. A new sleep study may be needed to confirm the type of sleep apnea you have. If your provider decides that you’re a good candidate for Inspire, they will refer you to an Ear Nose and Throat (ENT) surgeon.
three months to recover from the procedure. After four to six weeks, you will see your sleep health specialist to decide if it’s time to turn on your device based on the progress of your recovery. Once activated, you can easily turn the device on and off with a remote. Your provider will work with you to increase the voltage until it reaches a therapeutic level.
The ENT surgeon, also known as an otolaryn- gologist, and their team explain risks, benefits, and alternatives of the surgery. Careful consider- ation is given to ensure a patient qualifies based on approved FDA guidelines and insurance standards.
Ongoing monitoring: Once a year, a follow up is required with a sleep health specialist. An at-home sleep study may be needed periodically to ensure that your OSA is under control.
