SUPPORT FOR CANCER
Cancer Thriving & Surviving (CTS), you’re starting to see this slogan more and more often but what does it mean (and please note it means different things to different people).
Senior Health Partners, our agen- cy, believes in positive self-manage- ment and what we can do to help no matter what type of condition you’re facing. And as we have learned and continue to learn, there is a lot more to a diagnosis than just the diagnosis.
One way we support those who have been told they have cancer and those who have been affected by the diagnosis is to offer a six- week workshop that meets once a week, which teaches different tech- niques in handling day-to-day life. This workshop is in no way a form of treatment or meant to replace what you are doing with your med- ical professionals but just giving additional tools for the journey.
There are numerous topics cov- ered over the six week workshop including: 1) techniques to deal with problems such as frustration, fatigue, pain, isolation, poor sleep, and living with uncertainty; 2) appropriate exer- cise for regaining and maintaining flexibility, and endurance; 3) mak- ing decisions about treatment and complementary therapies; 4) com- municating effectively with family,
friends, and health professionals; 5) nutrition; 6) setting priorities; and 7) relationships.
You may ask yourself how did they get access to such an incredible workshop. The CTS workshop is an evidence-based best practice program that was developed through Kate Lorig, D.Ph. and Diana Laurent, MPH at Stanford University. It’s a program that was tested in the United Stated and the United Kingdom. And this program was tested by hundreds of participants to see what really works to make your journey better.
The workshop uses short lectures, lively discussion, brainstorming, relaxation techniques, guided imag- ery, problem solving, and action plans to engage participants in the self-management process. Each partici6pant receives a copy of the companion book, Living a Healthy Life With Chronic Conditions,
4th Edition, and a relaxation CD, Relaxation for Mind and Body. And currently this workshop is FREE and open to the public.

REPLACING YOUR SOCIAL SECURITY CARD ONLINE
We strive to provide you world- class service. With that in mind, Social Security rolled out a new ser- vice that allows some of our custom- ers to request a replacement Social Security card online.
The new online version of the Application for a Replacement Social Security Card allows people in Michigan to request a replacement card online through our secure my Social Security portal without travel- ing to a field office. You can create or log into your personal my Social Security account at www.socialsecu- rity.gov/myaccount.
Currently available in the District of Columbia, Michigan, Nebraska, Washington, and Wisconsin, it’s an easy, convenient, and secure way to request a replacement card online.
When you open a my Social Security account, we protect your information by using strict identity verification and security features. The application process has built-in fea- tures to detect fraud and confirm your identity.
In certain cases, security experts
at Social Security will contact you to ensure it is a legitimate application. We only issue a replacement card if there’s no suspicion of fraud and only mail it to a verified address.
To take advantage of this new service option, you must:
• Have or create a my Social Security account;
• Have a valid driver’s license in a participating state or the District of Columbia (or a state-issued identi- fication card in some states);
• Be age 18 or older and a United States citizen with a domestic U.S. mailing address (this includes APO, FPO, and DPO addresses); and
• Not be requesting a name change or any other changes to your card.
Before you request a replace- ment card online, you may want to consider whether you need to get a replacement card at all.
Most times, knowing your number is what’s important. You’ll rarely need the card itself, perhaps only when you get a new job and have to show it to your employer.
If you decide that, you do need a replacement card, log into your per- sonal my Social Security account and select “Request a Replacement Card.” Next, answer the screening questions to confirm eligibility, complete per- sonal data, and you’re done!
