 Designing the direction you will take in terms of care and housing into the future is in your hands. The sooner you start evaluating future needs the more choic- es you have. It’s always better to plan 20 years too early than one day too late.
If you have reached the age of tran- sitioning to Medicare it’s a good time
to re-evaluate your future choices from housing to health care. It’s time to review your financial outlook as well as update your legal documents and confirm who will play a roll in decision-making right alongside you into your future. If you have not started this process yet, then now is the time. Preparing early is a gift you give yourself as well as your loved ones.
Do not choose to ignore changes you are experiences for fear of having some- one else barge in and make choices for you. If you have prepared well, then your advocates will know what to do. Take hold of the power you have in preven- tive care and the plethora of choices
to consider regarding your future living arrangements.
The best news is that you can make all sorts of changes even while remaining in your own home. Whether its remodel- ing, bringing support services into your home, participating in Waiver or PACE, or possibly downsizing (rightsizing) to something more specific to your needs. Rightsizing might also open up funds for new activities or joining the Snowbirds down south in the cold months.
This directory looks at another cause for the need for care, a discharge from the hospital. The more successful you are at understanding and following directions the sooner you may be able to resume the anticipated level of activity. Turn to page 28 for suggestions.
When you or a loved one recognizes the need to start the conversation about future choices the article on page 14 provides suggestions. The sooner you start, the longer you may be able to remain in your own home with simple
adjustments. Be thoughtful about the decisions that are made regarding family and caregiving responsibilities. Includ- ing all adult siblings in the conversation removes frustration regarding choices. Continual communication is made easier with all the technology that we have gotten familiar with in 2020. Be sure to use it.
Change is never easy. The time to start looking for a long-term care solu- tion is before you need or want it. This will allow for the person ultimately need- ing care, as well as family members, to make decisions together. Making inten- tional decisions rather than ones that are limited by your options in a more urgent situation allows for the most comfortable transition.
Whether it’s in-home care or moving to a facility for support, you now have
a tool that can make evaluating your options easier. This guide breaks down the differences in care, how to cover the costs for care, as well as a checklist for you to develop when considering new options.
Maybe you’re reading this for a loved one. If so, it will help you to develop a plan for conversation and to recognize that the options can be overwhelming when a decision is thrust upon you. As an advocate, knowledge is key.
Be proactive. Plan ahead. Choose ahead. Document your choices. Re- evaluate and update when needed.
