When you become a DayOne patient, one of our Family Practice Doctors will be assigned as your primary physician and they will always have a special interest in you and your treatment. Occasionally you may be seen by another member of our team; be assured that your family physician will remain fully informed about
the status of your care. Physician Assistants and Nurse Practitioners are a valuable part of the DayOne Family Healthcare team. These licensed trained professionals work closely with your primary physician.
DayOne specializes in wellness exams, sports physicals, drug screening, vaccinations, diabetes & cholesterol & lipid check-ups, and treat acute conditions such as sore throats/ ears, headaches, cough & sinus problems, musculoskeletal problems (strains/sprains), acute urinary complaints, and laceration repairs.
We also assist with chronic disease management such as ADHD/ADD,
Anxiety, Asthma, CAD, CHF, COPD, Depression, Diabetes, Hypertension, Hyper-lipedema, Smoking Cessation and Weight Management Counseling.

a board-certified internal medicine and rheumatology physician? Dr. Thomas Ignaczak
specializes in Metabolic & Digestive Diseases, Arthritis–Psoriatic & Rheumatoid, Bursitis, Fibromyalgia, Gout, Lupus, Osteoporosis, Trigger Finger, and Vasculitis to name a few.
We also have
a board-certified general surgeon, Dr. Jeffery Mitchell who specializes in Mole & Skin Tag removal, Biopsies,
Colonoscopy, Debridement, EGD
for digestive disorders, Gall Bladder removal, Hemorrhoid banding, Hernia
repair, Ingrown nails, Lacerations, Lesion & Cyst & Lipoma removal, and Vasectomy.
DayOne’s Care Management Team consists of LMSW, RNs, LPNs, & CHW who are dedicated to helping patients with acute, chronic, or complex health conditions. Some examples are assisting with advance care directives, chronic disease management and education, community resources referrals, health coaching, medication reconciliation, medications assistance program applications, setting up in-home nursing
care for PT/OT & hospice care, substance abuse treatment referrals, transitional care management from hospital to home and back into primary physician care, support
for caregivers, veteran assistance and resources, and more. Medicare certified counselor on staff. Care Management is FREE of charge to our patients.
