CareWell Services SW serves seniors, persons with disabilities and their caregivers. Its mission is to promote health, choice, and independence. All services are provided in an environment of person-centered thinking which is a set of ideas, skills and actions that have as their purpose to assist individuals who use services to achieve greater control and influence.Since 1996, the Agency has been serving Barry and Calhoun counties through home and community based services to help older adults age with dignity in their own homes or the homes of their caregivers. The majority of those we serve are seniors aged 60 and over based on greatest social-economic need, including frailty, income, social or geographic isolation, or other factors that increases the risk for vulnerability. While the majority of those we serve are low-income, our services are not limited to those individuals only.In addition to its senior programs, CareWell Services is also a contract agent for the MI Choice Waiver program through the Michigan Department of Community Mental Health. This program provides support and services for adults with disabilities and frail seniors at risk of nursing home placement. Housing Options Counseling  Information &Referral/Assistance Medicare/Medicaid Assistance Program (MMAP) MI Choice Medicaid Waiver regarding Long Term Supports and Services for Home and Community Based Services Case/Care Management  Personal EmergencyResponse System (PERS) Options Counseling/Person Centered Counseling Senior Project Fresh/Market Fresh Produce Vouchers Elder Abuse Prevention/ Education ClassesRESOURCE LINE 1-800-626-6719MAIN OFFICE 269-966-2450REGION 3B AREA AGENCY ON AGINGPROMOTING HEALTH • INDEPENDENCE • CHOICECALL US,WE’RE HERETO HELP!PROMOTINGHEALTH INDEPENDENCE CHOICE200 W Michigan Ave Suite 102, Battle Creek, MI 49017 | | 269-966-2450MI Choice Waiver is a statewide program funded by the Michigan Department of Community Health.
